An Affidavit of Paternity (often a Voluntary Acknowledgment of Paternity) is a document signed by a father to legally recognize a child born out of wedlock as his own. This establishes the child's rights to inheritance, medical history, and financial support.
It is often signed at the hospital so the father's name can go on the birth certificate. Once signed, it has the force of a court order and can only be challenged within a short timeframe (usually 60 days) or upon proof of fraud/duress.
